STEP ONE: Pick the packet for your position:

• Combined Payroll Forms Packet for Professional Staff: Payroll Info Sheet, Direct Deposit Authorization, Professional Conduct Release, Verification of Employment, Authorization & Conditional Statement
• Combined Payroll Forms for DESPA and Individual Contract Staff

STEP TWO: ALL EMPLOYEES COMPLETE/READ THE FOLLOWING FORMS:

• Employment Eligibility Verification-Form I-9 (I-9 Instructions)
• Employee Federal Withholding Form – W4
• Employee Michigan Withholding Form-MI-W4
• Michigan New Hire Reporting MI-Form 3281
• New Hire ORS Retirement Plan Information

• The cost for fingerprinting is $69.00.
• Pay with Discover, Visa, MasterCard, or cash. American Express and personal checks are not accepted.

BRING: 

• Valid picture identification (e.g., Driver’s license, State I.D., or passport).
• Completed Live Scan form issued by the school, business or licensing agency that will receive your criminal history report.
• If being printed for EDUStaff, you need to bring an "Offer of Employment" letter from them.
